1. Diaper, wipe, maybe rash cream, and a pad to put the little guy on.
2. Lay the kid down. Get the pants, or what ever he/she is wearing, off so you can see the diaper. Pull the tabs off, exposing the genitals. Take the diaper off. Use the wipes to clean the front and the back. If a girl, make sure you wipe from front to back, as in from stomach down. Get the new diaper, and open it up. The picture goes on the front, and just slide the back under his/her behind. Pull the front between the legs, and put the tabs on. Wash your hands when you are done.
3. Beware of being peed on, as well as giving the kid a rash.
4. Depending on how old the kid is, a challenge can be getting them to stay still for the change. The younger ones like to role, while the older ones like to get away, as they would rather play than get their diaper changed.
