I don't think the order in which you sign or say the word matters as long as you repeat it frequently. Repetition is the most important thing. I have a day care in my home and I know when I'm teaching the kids signs, I just sign it as I say it and then give them the object or do what I have signed. After a few days of signing milk to a child and then giving them the cup of milk, I will wait for them to show me the sign before I give them the cup. Do the same thing with changing. Talk and sign and anytime you mention changing him make the sign. Whenever talking to your baby, sign whatever words you know as you say them.
